When does back pain need a spinal fusion such as TLIF? 🦴 TLIF or Transforaminal Lumbar Interbody Fusion is a spinal fusion procedure used in selected patients when the lumbar spine requires both nerve decompression and stabilization. TLIF may be considered for conditions such as: • Lumbar spondylolisthesis with instability • Lumbar spinal stenosis with instability • Degenerative disc disease in appropriately selected patients • Recurrent disc herniation with instability • Persistent nerve compression requiring decompression and stabilization During TLIF, the compressed nerves are decompressed, the affected disc is addressed, an interbody cage with bone graft is placed between the vertebrae, and screws and rods are used to stabilize the spinal segment while fusion develops. With a minimally invasive TLIF approach in suitable patients, smaller incisions and reduced muscle disruption may support earlier mobilization and recovery compared with traditional open approaches. Importantly, not every slip disc, sciatica or back pain patient requires fusion surgery. The decision depends on symptoms, neurological examination, imaging and evidence of spinal instability.
